These forms are specifically for use for student organizations and programs advised by the Center for Student Engagement. If another department advises your organization, please consult your advisor in that department for the appropriate forms.

Many of the following forms are available to download on the CSE Hoyalink Documents page and CSE Hoyalink Forms page. Student leaders are encouraged to download the forms directly from Hoyalink each time it is completed to ensure it is the most up-to-date version of the form.

Financial Documents and Resources

The following forms and documents can be found under the forms and documents tabs in hoyalink:

  • Invoice Request Form
    • This form is used to request payment from a vendor for services or products provided
  • Reimbursement Request Form
    • This form is used to request reimbursement from the university for organization purchases made with a student's own money
  • Journal Voucher Document
    • This document is used to initiate and complete payment between internal Georgetown organizations
  • OfficeMax Request Form
    • Organizations use this form is to request supply orders from OfficeMax
  • Gift/Prize Distribution Document
    • Students fill out this document upon winning a gift or prize from Georgetown University

Programming Forms and Resources

Publicity Forms

  • Broadcast Email and Student Affairs Slideshow Submission Form
    • If you would like to advertise an event for CSE weekly broadcast, fill out this form. Broadcast email messages go out every Tuesday. Messages may not concern elections or employment and must have clear campus-wide appeal. These are to be submitted the Monday before the broadcast at 12pm.

Travel Forms and Resources

Please note: these forms are specifically for use for student organizations and programs advised by the Center for Student Engagement. If another department advises your organization, please consult your advisor in that department for the appropriate forms.

  • Travel Form
    • A student leader (ie president, travel captain/coordinator) submits this form, which includes reason for travel, logistics, and attendees to notify the CSE that the organization is traveling. These are to be submitted the Monday before travel by 12pm.
  • Travel Waiver
    • All students travelling must submit this form before they travel, which includes local address, insurance information, and emergency contacts. 
  • Driver Authorization Information (Office of Risk Management)
    • After registering with edurisklearning and completing the "Driver Safety Training" course, students complete this form to finalize their driver authorization with CSE.
